Upload Issue

Summary: Uploading a file pops-up 2x so you can upload the file you want
Steps to reproduce:
- Create an issue
- add a comment in same issue
- browse a file
- pick a file
- Press OK
Expected behavior: It should only ask once about what file you want to be uploaded
Observed behavior: will be ask again to pick a file and changing the file changes the first pick you did
