Track Issue milestone changes

A record of issue milestone changes as events would be invaluable for workflow analytics; such an event would need to record the issue, the milestone, the date, and preferably the user as well. With this information, it is possible to calculate, for instance, how long an issue spends on average in a "Current Sprint" or "Testing" milestone.
