Improve GFM user docs

This has three points:
- make the hints in forms consistent and link them to the GFM help page
- fix the Markdown renderer options to also include hard_wrap option (see https://github.com/vmg/redcarpet) as advertised in the user docs
- overhaul the GFM help page itself
