Issue dashboard

This adds a dashboard for issues and merge requests assigned to yourself via /issues and /merge_requests.
See #173 (closed) for discussion details.
Please review this:
- Location / paths of /issues and /merge_requests
- The new 'user' layout
- Changes to the issues and merge_requests partials
All specs are green ;-)
